Passport Information and Application
The Iowa County Recorder’s Office is a passport acceptance agent for the
US Department of State Federal Passport Agency.
The friendly and knowledgeable staff at the Recorder’s office can assist U.S.
citizens, who meet the passport application requirements, with obtaining a U.S.
Passport Book or Passport Card.
The following must be submitted with your passport application: A. EVIDENCE OF CITIZENSHIP (One of the following)
1.
A certified copy of your birth certificate and/or previous passport.
The birth certificate must be issued from the state or the county, NOT
the hospital.
2.
Naturalization Certificate (will be returned with passport)
3.
Expired Passport (will be returned with passport)
B. TWO IDENTICAL PASSPORT PHOTOS
2x2 inches in size. Some digital photos are acceptable. WE CAN TAKE YOUR PASSPORT PHOTOS HERE IN THE
OFFICE FOR A COST OF $15.00 FOR THREE PICTURES. Please do not wear
white.
C. PERSONAL IDENTIFICATION
Valid Driver's License must be presented (NOT driving permit)
D. FEES FOR PASSPORT BOOKS - 16 & over (per person)

**Customers will receive their passports within 4-6 weeks from receipt of
completed application by Passport Agencies
F. EXPRESS MAIL - You may request mailing return of
passport by express mail. Return postage to the "U.S. Postmaster" will be $18.30
G. EXPEDITE FEE FOR PASSPORT - $60.00
**If you have not received your passport in 3 weeks, you may want
to check the status with the National Passport Information Center at
1-877-487-2778 or 1-888-874-7793 or www.travel.state.gov.Passport applications are accepted in the Iowa County, Iowa Recorder's Office
Monday through Friday between the hours of 8:30 A.M. to 4:00 P.M..
